Job Description:
Department/Unit: HBD - Neurology Work Shift: Day (United States
of America) Salary Range: $60,028.00 - $99,023.00 The Registered
Professional Nurse in the ambulatory setting provides care across
the life span to individuals, families, caregivers, groups,
populations, and communities. This is accomplished during
face-to-face encounters or through a variety of telecommunication
strategies, often resulting in the establishment of long-term
patient care relationships. During each encounter, the registered
nurse focuses on patient safety and the quality of nursing care by
applying appropriate nursing interventions, such as identifying and
clarifying patient needs, performing procedures, conducting health
education, promoting patient advocacy, coordinating nursing and
other health services, assisting the patient to navigate the health
care system, and evaluating patient outcomes. Ambulatory care
registered nurses, acting as partners and advisers within the
interdisciplinary team, assist and support patients and families to
optimally manage their health care, respecting their culture and
values, individual needs, health goals and treatment preferences.
Minimum Qualifications Education: Graduate of a professional
academic nursing program in which a Diploma, Associate Degree or
Bachelor’s Degree is conferred. Licensure, Certification &
Registration: Must hold current NYS Registered Nurse license.
Obtains and maintains certification in Basic Life Support (BLS).
Some departments require additional certifications. Experience:
Previous nursing experience in an ambulatory care setting or 1 year
nursing experience obtained outside an ambulatory care setting is
preferred. Skills, Knowledge & Abilities: Ability to improve job
performance through continuing education. Possesses knowledge of
quality concepts, principles, and problem-solving tools and
techniques and demonstrates ability to apply in support of
departmental and patient care quality improvement. Essential Duties

Demonstrates respect for individual differences of each person by

Practice, 3rd edition.) Assessment: Collects pertinent data and
information relative to the patient’s health or situation.
Diagnosis: Analyzes assessment data to identify patient needs based
on disease process. Outcomes Identification: Identifies expected
outcomes for a plan individualized to the patient. Planning:
Develops a plan that contains nursing interventions to achieve
expected, measurable outcomes. Implementation: Provides therapies
and instruction to address health promotion and prevention of
illness or injury. Coordination of Care: Coordinates care with the
patient, family, interprofessional team, and community based
resources to affect safe transitions in accordance with federal and
state regulations, utilizing the chain of command as necessary.
Patient Education: Delivers patient/family education with respect
to culture, values, developmental level, and readiness for
learning. Evaluation: Evaluates progress toward the achievement of
goals and outcomes. Standards of Professional Performance Practices
ethically. Practices in a manner that values cultural diversity and
the principles of inclusion. Communicates effectively in all areas
of practice. Collaborates with consumers of healthcare and other
caregivers in the conduct of care. Leads other caregivers within
the professional practice setting. Seeks knowledge and competence
that reflects current nursing practice and promotes future
thinking. Integrates evidence and research findings into practice.
Contributes to quality nursing practice. Evaluates one’s own and
others’ nursing practice. Utilizes appropriate resources to plan,
provide, and sustain evidence-based nursing services that are safe,
effective and fiscally responsible. Practices in an environmentally
safe and healthy manner. Patient Safety: Supports a system that
encourages the identification and analysis of errors and near
misses in the context of a safety culture. As assigned, conducts
departmental safety inspections to assure compliance with hospital
safety standards as evidenced by documented inspection reports. Is
aware of and compliant with all hospital and regulatory agency
safety goals and objectives Organizational Requirements: Maintain
strict adherence to the Albany Medical Center Confidentiality
