Hospice Case Manager Location: Barre VT Contract: 13 Weeks Shift:
Mon - Fri Weekly Pay: $2767 Summary: RN Hospice Case Manager
oversees and coordinates hospice care for patients with terminal
illnesses. Responsibilities include patient assessments, care plan
development, pain and symptom management, coordinating with the
hospice team, and providing education and emotional support to
patients and their families Requirements: •/tExperience with
hospice case management required •/tExperience providing hospice
care in a home setting required. •/tPrior experience with rural
travel preferred. •/tExperience with McKesson/Netsmart EMR
