Supervisor We are looking for a dedicated and compassionate
professional to join our team! This role is ideal for someone who
is passionate about delivering high-quality care while building
strong client relationships and supporting service growth. ?
Position Summary You will be responsible for coordinating and
supervising care delivery, ensuring client satisfaction, and
fostering meaningful relationships with clients and their families.
Key Responsibilities: • Conduct client assessments and develop
personalized care plans • Monitor and update care plans based on
client needs • Communicate effectively with clients, families, and
care teams • Address concerns and ensure overall client
satisfaction • Train and supervise caregivers, ensuring quality and
safety standards • Maintain accurate documentation and client
records • Perform home visits and reassessments • Coordinate with
external providers and healthcare professionals • Educate clients
and families to enhance quality of life • Support client onboarding
and decision-making process • Build and maintain strong, long-term
client relationships • Identify opportunities to expand services
and improve care delivery What We’re Looking For: ? Strong clinical
and interpersonal skills ? Experience in care coordination, home
care, or healthcare settings ? Leadership and team supervision
